The Strategic Forum Plant Safety Group has done a study of worker injuries and deaths that occurred as a result of machinery movement that created an unintended hazard. The movements followed an action involving either an accidental activation of a control switch or lever, or a deliberate activation which resulted in a movement that the operator did not expect, or else a circumstance where a guard or safety device had been removed or disabled (with or without the knowledge of the operator). The study focused on the design process and prevention practices needed before any incident has taken place as well as analyzing past incidents to determine the cause of each incident and what actions should be taken to avoid future repetition or any similar incident.
